Navigating the Self-Assessment Process

Approaching a self-assessment questionnaire with honesty and openness is essential. To accurately gauge the extent of a potential issue, it’s crucial to answer all questions truthfully, no matter how uncomfortable they may seem.

Consider the following factors while responding to the questionnaire:

  1. Emotional Aspects:
    • How do you feel when using substances? (e.g., craving, guilt)
    • Do you turn to substances when feeling depressed or angry?
  2. Frequency and Quantity:
    • How often do you use substances? (e.g., daily, weekly, monthly)
    • How much do you consume? (e.g., drinks per day or week)
  3. Consequences:
    • Have there been negative consequences due to substance use? (e.g., legal issues, relationship problems)
    • Have you faced legal trouble related to drugs or alcohol?
  4. Attempts to Quit:
    • Have you attempted to quit using substances, and were these attempts successful?

These details provide valuable insights for our clinicians, enabling them to design a tailored treatment plan that suits your specific needs.
